Active as of 2026-11-01 Computable Name: StructuredSigReviewFlagCS

Findings Structured Sig records on a Dosage it wrote from free-text directions, each telling a person what to check. Each code names what was found in the directions or the medication, and what it means for the Dosage. A Dosage carries every code that applies.

This Code system is referenced in the content logical definition of the following value sets:

This case-sensitive code system https://haau3.com/fhir/CodeSystem/structured-sig-review-flag defines the following codes:

CodeDisplayDefinition
unplaced-text Words not placed Some words of the directions were not placed in any element. The structure does not hold the whole instruction: review it against the text.
as-directed As directed The directions defer to instructions given elsewhere. The structure holds only what is written here.
taper-not-structured Taper not structured The directions change the dose or the timing over time, as a taper or as steps one after another. Only the text is given.
concurrent-not-structured Several instructions not structured The directions give more than one dose or schedule at the same time. Only the text is given.
ambiguous-frequency Frequency unclear The directions state frequencies for one dose that disagree, so no timing is given.
discouraged-abbreviation Do Not Use abbreviation The directions use an abbreviation or a way of writing a dose that is on The Joint Commission's "Do Not Use" list. It was read with its usual meaning: confirm that this is what the prescriber meant.
dose-unit-missing Dose has no unit A dose amount was written without a unit, and none could be added from the medication.
unit-from-product Unit added from the product The dose's unit was not written. It was added from the medication's dose form.
unit-form-mismatch Unit and dose form differ The dose's unit does not match the medication's dose form.
route-from-product Route added from the product The route was not written. It was added from the medication's dose form, which is given only that way.
route-form-mismatch Route and dose form differ The route written is not the way the medication's dose form is given. Confirm that this is what the prescriber meant.
medication-not-held Medication not in RxNorm The medication's RxNorm code is not in the RxNorm release, so nothing about the product was used.
ndc-not-resolved NDC not used The medication was named only by an NDC, which is not yet resolved to a product, so nothing about the product was used.
medication-not-read Medication not read The medication was named only in code systems Structured Sig does not read, so nothing about the product was used.
dose-form-unknown No single dose form The medication's RxNorm code names no single dose form (an ingredient, a brand name or a pack, for example), so the dose could not be checked against the product.
