Plans sold in a US county, in whole or in part, by the county's five-digit FIPS state and county code (12086, Miami-Dade County, Florida), bare or with the system http://terminology.hl7.org/CodeSystem/FIPS64, HL7's identifier for FIPS 6-4 county codes (the OID urn:oid:2.16.840.1.113883.6.93 is accepted too). A Medicare Advantage plan is sold in the counties of its service area in CMS's PlanArea file, and a regional PPO in every county of its region's states; a Marketplace plan in the counties of its issuer's service area in CMS's Service Area file, or every county of the state when the area covers the whole state. A plan sold in only part of a county matches the whole county. A plan's resource does not carry its counties: they are searched only. $counties lists a state's counties and their codes.
