The id and extensions a source sent for one entry of a repeating primitive (such as HumanName.given or Address.line) without saying which entry they belong to, because the source's list of values and its list of extensions had different lengths. Kept on the element that holds the list, with the position the source sent it at, instead of being attached to a value by guess. The source's own extensions are nested inside unchanged, under their own URLs. A record carries one instance per such entry.
